The Website is not a substitute for an in-person or telehealth consultation with a licensed healthcare provider. --- ## Telehealth Services Arbor offers virtual ("telehealth") consultations in addition to in-person visits at our locations. ## State Licensure Arbor's providers are currently licensed to practice medicine in the following states: Alabama, Arizona, Colorado, Georgia, Illinois, Indiana, Iowa, Kansas, Missouri, Nebraska, Oklahoma, Tennessee, Virginia, and Wisconsin. **Telehealth visits may only be conducted with patients who are physically located in a state where the provider is licensed at the time of the visit.** If you are located in a state where Arbor's provider is not licensed, we may not be able to provide a telehealth visit, even if you are an existing patient. You are responsible for accurately disclosing your physical location at the time of any telehealth visit. ## Informed Consent for Telehealth By using Arbor's telehealth services, you acknowledge that: - A telehealth visit is not equivalent to an in-person visit. Some examinations and procedures cannot be performed remotely. - Technical limitations (such as poor internet connectivity, audio/video failures, or device compatibility issues) may interrupt or affect a telehealth visit. - Your provider may determine that an in-person visit is necessary, in which case the telehealth visit will be ended and you will need to schedule in-person care. - You have the right to ask questions about telehealth and to refuse it without affecting your future care. - Information shared during a telehealth visit is treated with the same confidentiality as an in-person visit and is protected by HIPAA and applicable state law. - Telehealth visits are not appropriate for medical emergencies. If you experience an emergency during or after a telehealth visit, call 911. ## Out-of-State Patients If you are a resident of a state where Arbor's provider is not licensed, the Website's content is provided for informational purposes only and does not constitute the practice of medicine in your state. --- ## Health Information You Submit Through the Website Forms on the Website may collect health information (also called "Protected Health Information" or "PHI" under HIPAA).
